ANILTTA ALEX BS, BSN, MSN, APRN a board-certified adult nurse practitioner (ANP-BC) has over 16 years of nursing experience and expertise in geriatric care. Aniltta graduated with a Bachelor of Science degree in Biology from the University of Illinois in 1997 and completed her Bachelor of Science in Nursing from North Park University in 2000.
She has experience working as a staff nurse in orthopedics, intermediate and ICU units and has taken positions of leadership including nursing preceptor, nurse educator, and charge nurse. Aniltta completed her Master of Science in Nursing and adult nurse practitioner certification at Loyola University Chicago in 2006.
She is a member of Gerontological Advanced Practice Nurse Association (GAPNA). She has also been involved in Dementia and CHF research studies and in 2015 presented at the American Health Care Association/National Center for Assisted Living (AHCA/NCAL) convention regarding “Closing The Gap on Hospital Readmissions”.
Aniltta states, “Nursing has allowed me to serve patients, listen to their needs and help them achieve their healthcare goals. Being able to provide direct patient care and seeing the difference I can make in my patients’ health is the most rewarding part of my career.”
PATRICIA McCANN BS, RN, MSN, APRN is board certified as an adult (ANP) and geriatric (GNP) advanced practice nurse (APN) and has 19 years of nursing experience in acute care, primary care, and skilled nursing facility (SNF) setting.
As an APN, Pat has provided care to geriatric patients in the home setting, assisted living facility, sheltered care, primary care and most recently SNF settings.
She is currently responsible for clinical management of post-acute patients and interfaces with the interdisciplinary team, administrators, families and physicians, to provide comprehensive, quality care and positive outcomes. Pat received her Bachelors of Science in Nursing from St. Xavier University in Chicago and Masters of Science in Nursing from the University of Illinois College of Nursing in Chicago.
She is an adjunct professor at the University of Illinois College of Nursing in Chicago, past president of the Chicagoland Geriatric Advanced Practice Nurses a chapter of The Gerontological Advanced Practice Nurse Association (GAPNA) & presented a lecture entitled “Antipsychotic Medication Use in Illinois SNF: A Rational Approach to Change” at the GAPNA National Conference in Phoenix Arizona in Sept 2016. She is also a member of AANP & AMDA.
Pat’s interests are in health care systems, dementia care and mentoring new nurse practitioners.

Effective transitions of care are critical for older adults with chronic disease who seek medical help in various healthcare settings. Lotus Geriatric Nurse Practitioners work to improve care transitions by providing medical management and coordination of care across the patient care continuum.
Patients discharged from the hospital are followed into post-acute care and chronic care settings. Our nurse practitioners work collaboratively with acute care, primary care, and home health providers to deliver high-quality patient care, reduce avoidable hospital readmissions, adverse events, and health care costs.
Lotus Geriatric Nurse Practitioners work with the multidisciplinary team as well as establish a trusting relationship with patients/families to coordinate care. We provide transitional care visits for high-risk patients within seven days of discharge from the Skilled Nursing Facility and continue to medically manage chronically ill patients in the home. Progress notes are also faxed to primary care physicians to promote communication and warm handoffs among providers.
